Answer a few questions about your health history, select a date and time, and submit your form to schedule an appointment with a medical marijuana doctor for your evaluation.

APPROVAL

Once approved, you will receive your Doctor’s recommendation immediately electronically via email, along with instructions on how to register with your State Health Department.

ACCESS

If you are approved for medical marijuana, access any dispensary with your Medical Marijuana Card. Our support team is available 24/7 to assist you after you complete your evaluation.
